Before you fill out this form to register on our site, please download the Photo/Video Release & Privacy Policy Signature Form, print it out, sign it, and either scan or take a photo of the signature page so you have it on hand to upload during the registration process.

Once you are registered, login, and under your Account menu, you will find a link to the "Current Scholarship Application".
